Shepard Park Home Care is owned and operated by StuartCo and provides services to The Wellington 24 hours per day/7 days per week.  As a team we strive to provide the best possible care to our residents.  Shepard Park Home Care is licensed by the State of Minnesota as a Comprehensive Homecare Provider.

Highlights include:

  • 24-hour on-site home health aide staffing to provide personal care based on the individual care plan established
  • Registered Nurses (RN) on-site during normal business hours at both communities and on-call outside of these hours.
  • Licensed Practical Nurses (LPN) most evenings and weekend days at The Wellington and evenings and overnight at The Alton.
  • RN assessment, review and admission to home health services upon move in and when change in condition occurs
  • RN coordination of physician requests, laboratory and other health care professionals
  • RN visit every other month and maintenance of clinical record
  • Response by on-site home health aides to unanticipated calls through the nurse call systems 24 hours a day/7 days a week
  • Monthly Wellness Clinic.

Sodexo provides dining service in a restaurant style atmosphere at The Wellington.  Dining is open at the Wellington from 6:30 a.m. until 7 p.m. – 7 days a week. The goal of the dining experience is to not only offer a creative menu but to allow for additional choices with an always available menu at The Wellington.  Through Sodexo we have access to a Dietitian to assist with dietary needs, questions, special diets and provide education.  We encourage family and friends to join us for any meal.  We also have catering for special events that a resident or family member might like to engage in at our communities.

Bluestone

Bluestone Physician Services provides on-site primary care services for residents living in assisted living, memory care or group home communities. The physician-led teams visit the patients on a regular basis and collaborate with the communities staff, patient and patient’s family to provide customized medical care. Interim

Interim HealthCare offers skilled home care services, hospice and private pay services should and when the need may arise.  This allows residents to age in place.

As residents age, they frequently lose muscle tone and dexterity.  An on site therapy clinic offers physical, occupational and speech therapy under the resident’s Medicare Part A and B plans, assuming qualifying criteria is met.  This comes at no (Medicare A) or very little (Medicare B) cost to the resident as they work to increase function, range of motion and address pain issues.

Better physical health often improves emotional health which, in turn, can increase longevity and quality of life

Proactive approach to care that addresses weakness, unsteady balance, disease progressions, and other conditions which can lead to crisis situations or hospitalizations.

Physical Therapy Assistant on site to ask questions about skilled services and Medicare/Insurance benefits on a daily basis, Monday through Friday.

The onsite Physical Therapy Assistant is trained to identify when residents qualify for Medicare Home Health Care (RN, PT, OT, ST, HHA, Hospice) and and offers education on safe transferring, balance, home safety, exercise and activity.  Baseline screens and safety checks are available for all new residents or if there has been a change in condition.

eldermark
The Wellington has implemented certain applied technology to ensure our workforce is enabled to provide the best and safest care to our residents. We have adopted Eldermark Software as our electronic health record (EHR) to achieve these goals. The Eldermark technology is moving us toward a paperless and efficient clinical management system. The various modules of this technology provide for real-time care data populating the resident clinical record and a real-time connection to our primary pharmacy. With real-time information in the EHR, our nurses and our resident’s physicians have access to the most immediate clinical data in the health record about residents to ensure the most informed care decisions. Linked to our billing system, Eldermark’s EHR assists us in creating accurate invoices and service records.

With our implementation of Eldermark technology, we are already prepared for the time when all the other providers of care to our residents are also ready to share health information electronically with each other in  secure manner, enabling the highest potential for collaborative and coordinated care – all of this to optimize each resident’s well-being.

The Wellington partners with EduCare to make quality training tools available to our team.  Customized for The Wellington, EduCare courses focus on teaching industry best practices, regulatory compliance requirements, and practical tools for implementation.  From training on memory care, aromatherapy, and customer service to practical skills such as medication administration, infection control, fall prevention, housekeeping, and dining, the list is vast offering the team at The Wellington many unique learning opportunities.
